Placements at Wild Minds Alternative Provision are referral only. The referral form below is the first point of contact for schools, local authorities and professionals. Once a referral is submitted, we email the referrer to arrange a visit.

Suitability check
Review our provision, age ranges, needs and environments before referring.
Referral
A school, local authority, college or involved professional completes the form.
Supporting documents
We request the information needed to understand needs, risks, outcomes and current support.
Review and capacity
The team reviews suitability and current site capacity, then responds to the referrer.
Visit and assessment
The young person and family meet the team and see the proposed environment.
Proposed start
If the placement is suitable and agreed, we set out the plan and a supported start.

This form is the first point of contact for local authorities, schools and professionals. You do not need every detail to start. Initials, age and a short summary of need are enough for us to begin. Once the form reaches us, we email you to book a visit.

Can a parent or carer refer directly?
No. A school, local authority, EHCCO or involved professional needs to complete the referral form. Families can send them the link to this page.
Do I need every document before referring?
No. Initials, age and a short summary are enough to begin. We will ask the referrer for relevant supporting information during review.
Does a referral guarantee a place?
No. Every referral is reviewed for suitability, safety and current capacity before a visit or proposed start.
What happens after submission?
We email the referrer. If the referral looks like a good fit, that email includes the next steps for arranging a visit and assessment.
